AUSTIN/TRAVIS COUNTY COORDINATED COMMUNITY PLAN TO END YOUTH HOMELESSNESS

In January 2017, Austin was one of the first 10 communities in the United States to be awarded Youth Homelessness Demonstration Program (YHDP) funding by the U. S. Department of Housing and Urban Development. This grant supported LifeWorks, the Ending Community Homelessness Coalition, and more than 100 community partners in developing and implementing a coordinated system strategy to end youth homelessness.

OPERATIONALIZING THE EFFORT TO END YOUTH HOMELESSNESS

This toolkit provides a set of proposed outcomes and data elements to measure organizational or community-wide progress toward meeting the core outcomes for youth experiencing homelessness, as outlined by the United States Interagency Council on Homelessness.

TARGETED ASSERTIVE OUTREACH: A COMMUNITY HOW-TO GUIDE

This handbook provides a step-by-step guide for conducting Targeted Assertive Outreach.  This innovative and systematic approach to outreach can be used to better understand the scale of youth homelessness in one’s community, thus supporting systems-level planning efforts and service delivery at the individual level.
